According to the American Society of Plastic Surgeons, more than 300,000 breast augmentations were performed in the U.S. in 2018, making it the most popular cosmetic surgery. In general, most women — including the majority of my patients — are pleased with the fuller, shapelier, sexier silhouette they can achieve with breast implants.
For decades, women have wanted larger, fuller breasts and have chosen breast implants as a safe, effective way to get them. These days, however, that trend is reversing in many cases. Women’s lifestyles are changing, as well as what our current culture considers to be attractive and desirable.
Today, smaller breasts that look more natural and realistic are becoming more popular. For this reason, many women are choosing to have breast revision surgery, either to have their breast implants removed or to replace them with a smaller size. When it comes to breast size, many people assume that women only want to go larger. However, in my plastic surgery practice many patients come to me wanting their breasts to be smaller. Very large breasts, especially on small-framed women, can cause a lot of physical discomfort: neck pain, back pain, indentions on the shoulders from heavy straps, skin rashes and problems with exercise.
Overly large breasts can also make it hard to find properly fitting clothing and bathing suits, and for many women, can make them uncomfortably self-conscious. If this sounds familiar to you, breast reduction surgery might be the welcome relief you’ve been looking for.

Until now, if you wanted larger breasts, you only had two choices of implants:
- Silicone implants that would give you natural-looking results but you'd worry about silent rupture.
- Saline implants that would give you peace of mind but you'd worry about wrinkling and an unnatural feel.
Now there's a third choice: the structured Ideal implant
Earlier this year, textured breast implants made by the manufacturer Allergan were recalled in the United States and around the world because they have been linked to a rare form of cancer called breast implant-associated anaplastic large-cell lymphoma. Allergan is now offering to replace their recalled BIOCELL textured breast implants with Allergan smooth implants at no charge.
For many years, if you wanted larger, fuller breasts, you had two options: implants filled with silicone or implants filled with saline. Today, you have a third choice: the structured Ideal implant, which combines the natural look and feel of silicone gel with the safety and peace of mind of saline. Now you don’t have to choose between a beautiful, natural look and the confidence of knowing your implants will stay intact.
Now there are three types of breast implants
You may have read or heard in the news that the breast implant manufacturer Allergan has recalled its textured breast implants, here in the U.S. and around the world. A July 2019 statement from the FDA explains the recall:
To protect individuals from the increased risk of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), associated with Allergan BIOCELL textured breast implants, the Food and Drug Administration (FDA) requested that Allergan recall its BIOCELL textured breast implants and tissue expanders. Allergan agreed and is removing these products from the global market.
(FYI: BIA-ALCL is not breast cancer. It is a type of non-Hodgkin's lymphoma, which is cancer of the immune system.)

What is a “gummy” breast implant?
One of the joys of being a plastic surgeon is being able to give women the size and shape breasts they desire. For some, this means larger breasts (breast augmentation). For others, it means smaller breasts (breast reduction) or reconstructed ones following a mastectomy (breast reconstruction). For those who want bigger, fuller breasts, breast implants are a safe, highly successful way to achieve the breast size they’ve always wanted. Recent concerns about textured breast implants
There are two general types of breast implants: textured and smooth. In my plastic surgery practice, I only use smooth implants but some plastic surgeons, especially in Europe, have used textured implants that have recently been recalled in the United States and worldwide.
